New feature - Phone number field in Live and Offline widget.

1. Customize Pre-chat survey form fields - You can make them optional or required.
2. Collect Phone number from your visitors.(Both Live chat and Offline chat widget).
3. Provision to add/update the phone number from the Agent chat window.
4. View phone number information in your History- Chat transcript.
